Your GP manages your healthcare when you are at home. They have an important role to play in the longer term management of your health, which may be affected by your cancer or its treatment. Your GP can make referrals to community services that can provide ongoing support.  The availability of the different community services may vary, depending on where you live.

Social Prescribing teams work closely with GP practices in both Sunderland and South Tyneside to enable them to become happier, healthier, more socially connected and empowered in their own lives.
