Annual Self Certifications

NHS Foundation Trusts are required to self-certify against a number of specific statements whether or not they comply with the conditions of the NHS provider licence (which itself includes requirements to comply with the National Health Service Act 2006, the Health and Social Care Act 2008, the Health Act 2009, and the Health and Social Care Act 2012, and have regard to the NHS Constitution).

Declarations of Interest

The Register contains, as they become available, the declaration of interests made by Executive and Board members.  A separate register is maintained for Governor declaration of interests.

In the interests of full transparency, staff are required to comply with our Standards of Business Conduct Policy which sets out how and when to declare interests, gifts and hospitality. Decision-making staff are required to make an annual declaration, including a nil declaration, even if they have no interests to declare. 

All staff are required to make positive declarations on receipt of any gift or offer of hospitality or if they have any known conflict of interest that may be a conflict to them and the Trust.
