Share your feedback - Friends and Family test

Understanding the experience of our patients, service users, carers and relatives is really important to us. Your feedback helps us to understand what we do and what we could do better.

Our Patient Experience survey, which includes the Friends and Family test question, is available for you to complete during any point of your care. This type of feedback is called the Friends and Family test because it is based on asking all patients the following standard question: 'Overall, how was your experience of our service?'

The survey can be completed in a variety of ways:

• QR codes
• Website links
• CIVICA app on Trust iPads
• Paper survey

Postcards with the QR code or web link to leave feedback for the service received may be offered to you at the end of your visit or be included in information given to you by our staff. If you would prefer to complete the survey in paper format or require some assistance to do so, please ask a member of staff. Details can also be found on posters and postcards across our Trust sites (insert images of posters).
