The Patient Safety Incident Response Framework (PSIRF) is used for patient safety investigations. We started using it at STSFT in November 2023. It replaces the Serious Incident Framework.

PSIRF helps us create a positive patient safety culture. It will help us to understand how and why a patient safety incident or event has happened. This helps us to learn and improve to make care safer for patients. 

When something goes wrong, we focus on four key topics: 

  • Compassionate involvement
  • Proportionate response
  • A system approach 
  • Supportive oversight 

This helps us to learn and we can reduce avoidable harm and keep making our care safer.
