Inpatient care
The heart failure nursing service is able to offer inpatient support to those with a diagnosis of heart failure by providing:
- Symptom review and providing individualised management plan

- Post discharge from hospital a two week review will be organised for an outpatient clinic or home visit

- Optimisation of medical therapy
- Patient Education
- Heart Failure MDT
- Dedicated Sacubitril Valsartan clinic
- Referral out to the advanced heart failure service at Freeman Hospital
- Heart Failure Rehabilitation

- Palliative Care Support
