Our Occupational Therapy Team within Acute Medicine use therapeutic interventions to achieve a safe and timely discharge for our patients. This may include a referral to community teams for re-enablement in your home or a bed-based facility.
The team carry out assessments using a holistic and patient-centred approach and work closely with a multi-disciplinary team and external agencies to carry out a wide range of assessments including:
- Risk assessments
- Assessing for and providing equipment
- Making recommendations for carer support to maintain/improve a person’s quality of life
